Student Form for Re-Requesting Diploma

Please complete the form below if you wish to request your diploma again.

After completing the Diploma Request Form, it must be submitted along with the required documents to the Faculty Student Affairs Office.

Submission can be made in person or sent by post to: Uludağ University Faculty of Medicine, Görükle Campus, 16059 Nilüfer / BURSA, Turkey.
